OT Evaluation
Includes a 30 minute parent telehealth consult, 90 minute in-person evaluation, thorough documentation, and 30 minute parent/ OT debrief to review results.
$325 total
OT Sessions
Each 60 minute in-person session, documentation, and superbill.
$150 per session

Time and location will be a collaborative effort between both your family’s and my availability.

No, you do not need a referral from the doctor. However, if you choose to submit to insurance, it is often helpful to have a referral for OT from your child’s pediatrician or other relevant medical provider.

All individual evaluations and sessions are private pay. I do not accept insurance. I am able to provide thorough documentation and a superbill at the completion of each session if you wish to submit for insurance reimbursement.
